If your club has its own website rather than an SCM Website (Option 2), you can embed an SCM trial request form on it. Visitors complete the form on your website, and their submissions appear in your usual Trial Requests list. The Public Forms tab is available to club managers and users with the Edit Club Details permission. At present, this tab offers trial request forms; waiting list forms are not available yet.
Set up the form
- Go to Setup > Club > Details and open the Public Forms tab.
- Set Minimum age in years, or enter 0 for no minimum age.
- Choose the Disciplines applicants can select. These choices come from your club's enabled disciplines under System Features. Leave every discipline unchecked if you do not want to ask for one.
- Under Custom questions, choose an existing custom form if you want to ask extra questions. You can create or edit a custom form first, then return to this tab. Choose None if you do not need extra questions.
- Enter your website's address under Approved website origins, one per line. For example, enter
https://exampleclub.co.ukandhttps://www.exampleclub.co.ukon separate lines if visitors use both. Include only the scheme and hostname (and port if your site uses one): no page path, trailing slash or wildcard. One origin covers all pages on that site. - Optionally add a plain-text Introduction above the form and a Confirmation message to show after submission. Enter an HTTPS Club privacy-policy URL; visitors will see a link to it on the form.
- Tick Enable public trial requests and select Save trial form. A success message confirms that your settings were saved. You can leave the form disabled and save a draft while you finish setting it up.
Saving creates a Public form ID and displays the Embed code. Your saved settings remain on the tab when you return later.
Put the form on your website
Select Copy embed code, then paste the entire code into an HTML block on the page where you want the form. In WordPress, use a Custom HTML block. Keep both parts of the code together. Publish the page and open its live URL to check that the form loads.
Your website editor must allow script tags. If it removes the script, ask your website administrator how to permit the embed code. If your site uses a Content Security Policy, it must allow the SCM host used in your embed code for scripts and frames.
Submit a test request using details you control and check that it appears under Club > Trial Requests. The form includes a security check and SCM's spam checking. Applicants receive the usual confirmation email, and your club's existing trial request workflows apply.
Review submissions
Open a request to see its Custom form responses, including any answers to your selected custom questions. They also appear on the printable trial request. If you copy the request to the waiting list, the responses go with it. When you convert that waiting list entry to a full member, the completed custom form appears on the member's Documents tab. Keep the custom form and its questions in place until you have converted any waiting list entries using them.
If the form does not load
Check that Enable public trial requests is ticked, that you saved an HTTPS privacy-policy URL, and that Approved website origins contains the address shown in your visitor's browser. www and non-www addresses need separate entries. If the form is blank or blocked on your website, check whether your website editor removed the script or your site's Content Security Policy blocked the SCM script or frame. If saving reports that server settings are missing, contact SCM support.
Use Rotate public form ID only if you need to invalidate the existing embed. After rotating, copy the new embed code to every page using the form; the previous code will stop working.
